A Child ID Card is a valuable tool for locating missing children and identifying found children.

• Focuses on distinguishing characteristics such as birthmarks and/or scars, physical and/or mental disabilities

• Name, address, phone number, nickname, date of birth, height & weight, hair & eye color

• Dental and medical office information

• Name, address and phone number for parent(s) or guardians

• In addition to all pertinent information, a lock of hair and a current photograph (a polaroid is taken at time of appointment) may be attached so the parent's won't need to stress over trying to locate requested items.

Additional Card(s): $10.00 each

• 5 - 10 miles $40.00

• 10 - 20 miles $45.00

• 20 - 30 miles $50.00

• 30 - 40 miles $55.00
